Collaborative care programs are one approach to integration in which primary care providers, care managers, and psychiatric consultants work together to provide care and monitor patients' progress. These programs have been shown to be both clinically-effective and cost-effective for a variety of mental health conditions, in a variety of settings, using several different payment mechanisms.
An interdisciplinary, evidence-based approach to whole-person care.
The Collaborative Care Model (CoCM) uses a team-based, interdisciplinary approach to deliver evidence-based diagnoses, treatment, and follow-up care to an identified patient population. It is being embraced and adopted in several health-care systems and payors across the state.
Not only does it provide evidence-based care of mental illness and substance use disorders, it is documented to improve outcomes, satisfaction, and access โ by bringing behavioral health expertise directly into the primary-care setting patients already trust.

CoCM works because the team works together around a shared focus โ the identified patient โ sharing the same record, the same goals, and the same accountability for progress.
Primary Care MD/NP/PA Practitioner
PCP
Initiates referral to behavioral health, prescribes medications based on collaborative care recommendations, and provides treatment as part of the patient-centered medical home. Refers and engages the patient in CoCM, manages medication recommendations, and oversees outcomes.
Behavioral Health Manager
BHCM
Provides care management and support to patients and team. Tracks the defined patient population in a registry, paying close attention to patients who are not improving. Provides brief therapy and educates the patient about diagnosis and treatment.
Psychiatric Nurse Practitioner
Consultant
Provides regular consultation to the PCP and BHCM and recommendations for treatment, focusing on patients who are not improving. Empowering's Care Manager Consultant works alongside the treating-billing physician.
A team of three individuals delivers CoCM, with the patient as the central focus โ care management for those receiving behavioral health treatment and regular psychiatric specialty consultation.
